Process of Creating a Care Package

Taking the first step

We know that making the decision to have care workers can sometimes be daunting and confusing, so we have made a step by step guide to help you.

Initial Meeting

An initial meeting will be arranged to assess and discuss your needs and requirements with our care co-ordinator. If you prefer, you may have a friend or relative present at the meeting. You will be consulted fully regarding the type of care package you require. Issues to be discussed will include personal care and well-being, continence, mobility, medication support, personal safety, dietary needs, family involvement and communication concerns. Your care schedule and service requirements will be discussed. At this point a quotation will be provided with the cost of your care needs. This first assessment will be free of charge.

Assessment of Care Needs

If you wish to proceed further, an appointment will be made to visit you at home to finalise your care needs. This appointment will include a review of the home environment, any mobility issues, equipment requirements, level of medication assistance, and any dietary and nutritional needs. Suggestions may be given to you regarding any adaptations or equipment that you may need to ensure your safety and wellbeing at home whilst at the same time providing a safe work environment for our care workers.

Your Care Plan

The care co-ordinator will then create your care plan. This will detail any relevant personal information such as health issues, hearing, sight and cognitive abilities, continence and toileting needs, religious beliefs, GP and next of kin contact details. Your care plan will also detail your care needs and preferences, and what is to be expected of our care workers at each call. A copy of this will be given to you to be reviewed and any necessary changes will be made. Once agreed, your care package will now begin.

Reviews

After 72 hours of your care package commencing, a review will be made via phone call to check in with you and ensure you are happy with your care. The care co-ordinator will visit you after six weeks to discuss any changes in your needs and preferences that may have arisen. An annual meeting will be arranged to carry out a full review of your care package. This will be similar to your initial assessment. However your ongoing requirements will be continually monitored by your care workers and our care co-ordinator. If you wish to amend your care package at any time, please do not hesitate to contact us.
